    Counter Notification – Request for Restoration
    If your content was removed, you may submit a counter notice to request its restoration. Your counter notice should include:

    1. Your physical or electronic signature;
    2. Description of the removed material and its original location;
    3. A statement under penalty of perjury that you believe the removal was due to mistake or misidentification;
    4. Your name, address, and phone number, and a statement consenting to the jurisdiction of the appropriate federal court.
